Sandstone Care tracks patient-report symptoms and treatment engagement using four psychometrically validated scales, administered weekly or every other week. Scores are automatically charted in real time within each patient’s medical record. This allows staff to monitor progress, adjust care as needed, and review findings alongside patients to strengthen their trust and collaboration with their treatment team.
Depression and anxiety are the most common co-occurring conditions among people seeking treatment for substance use and mental health disorders. Routinely tracking these symptoms gives clinicians actionable insights into treatment progress and helps guide treatment decisions.
The Patient Health Questionnaire-9 measures depressive symptom severity across five levels (Kroenke et al., 2001).
The Generalized Anxiety Disorder-7 scale measures anxiety severity across four levels (Spitzer et al., 2006).
Therapeutic alliance, or therapeutic rapport, is the relationship patients develop with their treatment team, encompassing trust, connection, and unconditional positive regard to reinforce healing and recovery. Therapeutic alliance is strongly associated with treatment engagement and success, making it a critical component in routine treatment tracking.
At all levels of care, the impact of therapeutic relationships on treatment success is crucial. That’s why patients at every level of care complete the Working Alliance Scale, Revised (WAI—SR; Munder et al., 2010) on a weekly basis to monitor the developing strength of their therapeutic relationships with the clinical psychotherapy team.

During Medical Detox and Residential Care
Detox and Residential inpatient treatment include the 24-7, hands-on, therapeutic support of nursing and behavioral health professionals (BHT) who play significant roles in our patients’ healing. Patients complete the Scale to Assess Therapeutic Relationships (STAR—P; McGuire et al., 2007) on a weekly basis so that nurses and BHTs can gauge the progress of each patients’ therapeutic alliance.

From the onset of care, therapeutic alliance was strongly established. When patients feel safe, validated, and supported, they are more likely to be engaged with their treatment. Those with stronger bonds also were more likely to report greater reduction in depression and anxiety.
Learn MoreIn 2025, Sandstone Care treated 3,487 patients across the United States. Half of our patients were emerging adults (between ages 18 and 30) with an average age of 25.4 years. Half identified as cisgender male, 43% as cisgender female, and 6% as nonbinary.
Across programs, the most common mental health diagnoses treated were depression and anxiety. Substance use treatment leaned heavily towards alcohol, cannabis, and at the Detox level of care, opioids.
